Why do some years a derm residnecy will match like 1 more person? so for example if there are 4 reisdency matches to that program one year, 5 the next, and back to 4?

Because some programs add a special spot just for a specific applicant. Did an away and a resident told me their PD pulled HEAVY for them and got the other PD to create a spot (funding availability too)
it depends on a few things, but usually a program just wants a specific person (often it is an RY candidate who already did their PGY-1 and is looking for a PGY-2 spot). This means oftentimes programs will have 5 one year then 3 the next (if its a 4 resident program) or 5 then back to 4, etc. Lots of niche details but that's the gist afaik
It's largely tied to funding, typically you will see programs alternate between some number plus or minus one every year
